Grade 4 hydronephrosis in left kidney and nephrolithiasis in right kidney
Hello everyone I am devastated
My cat is a 6 years old, spayed female, fully vaccinated. I want to prepare myself for everything and get more opinions.
Three days ago, she vomited and refused to eat so we rushed her to the vet. An X-ray showed an unusual kidney size and two stones. During our visit 3 days ago, the vet gave her two injections and sent us home to monitor her. She ate a little and passed some stool.
Before this suddenly happened, she was completely fine and healthy. Her appetite, water intake, litter box habits, energy levels, playfulness, and sleep routine were all perfectly normal. She showed zero signs of pain or illness. However, over the past 3 days, both her water intake and urination frequency increased significantly (she urinated around 7 times in 24 hours). We had another X-ray today to see if the stones had moved, but unfortunately, they hadn’t.
She stayed at the clinic today for bloodwork and an ultrasound, and the results came back pretty rough.
Left kidney: Grade 4 Hydronephrosis. The vet says we have lost function in this kidney and recommended an immediate surgical removal.
Right Kidney: Two small nephroliths (one measured at 0.34 cm in the renal pelvis). It is currently not blocking urine flow.
Labs & Results:
Creatinine: 2.70 mg/dL BUN: 37.4 mg/dL fSDMA: 26.4 ug/dL
Our primary vet referred us to a specialist in another city. I am traveling with her right now, and we have an appointment tomorrow at 10:30 AM.
I’m terrified and trying to prepare myself for what’s ahead.
Any advice, success stories, or encouragement would mean the world to me right now. Thank you.

Vets seem to have two modes: "wait and see" or "emergency now." Hope your kitty pulls through, keep us posted.
