I'm a traditionally feminine cis woman and got called "sir" by possibly the oldest gentleman I've ever seen
I think sometimes their hearing or sight fails them

People use to say "Sir" to show respect for ones position, typically in military but can also be for police, railroad workers and other ranked type jobs. Not very common.
