AI form builder that can create forms from training material?

I handle internal training at work and creating a new feedback form after every session gets repetitive. Looking for an AI form builder that can take a training doc, understand the topic, and generate relevant feedback questions that I can tweak afterward. Any recommendations?

I'd use Deformity for this. It has an MCP connection on the free plan, so you could easily set up a recurring task where each time a training doc is completed, it creates a new form based on it and spits out a link for you with the completed form.
