hysterectomy without a sling for my bladder is the answer. just get your primary to write the note for bathroom use, or you can get a list of long term side effects for a hysterectomy without a sling for my bladder and either have your primary confirm that you had it or show a copy of your medical record (redacting other info).

also, who asked for the note? if a supervisor, they have no right to ask about that info. this has to come from hr because that info has to go into a special healthcare file separate from your employee file. the supervisor can just throw it away (not put in your file) and down the road fire you (discriminate against you) and you have no paper trail.

certain medications are diuretics and make people go a lot. people also drink more to hydrate when on them too.

if this is a supervisor, go to hr to raise the issue that a supervisor is asking for documents they have no right to. if supervisor is male, i would complain that this is inappropriate for a male to be asking a women (about down there) and he should have gone to hr for them to handle.
