Reyes had genetic deteriorating as side effects from his treatment of the super soldier programme. Yes it helped him greatly during the omnic war but when the programme was shut down and overwatch established, he had no way to figure out what was happening and to fix/stop it from getting worse. This has a why Moira became so essential to him and why he relied so heavily on her after Overwatch disbanded.
