Its an old sign. The update about "uncle died" is very clearly taped over the original. 

If only that surgeon had been trained by Dr. Milford Watkins, the beloved uncle would still be with us.

Well ofc, they are required to take it! The problem is when these surgeons get so overconfident in their dagger training that they swap out their scalpel!
