Treatments My method: Iontophoresis water w/ Qbrexza (glycopyrrolate)

Hello! I thought I would share my iontophoresis method in case it helps others, as I've never seen it before.

TL;DR: I mix 1-3 Qbrexza wipes per 0.5-1 Liter of hot water & use that water for my iontophoresis. This results in the ionto being more effective and lasting longer. My typical routine is 1mg glyco daily + iontophoresis every 4-6 wks. Crushed glyco pills work too but their doseage is so low that it ended up not being worth it to me.

Check below for the concentration table and warnings.

Long version:

I have had primary palmar, plantar, and underarm hyperhidrosis since childhood (since age 6-8, at least). I have tried Carpe (expensive), Drysol (gave my skin a reaction), Oxybutynin (stopped due to concern over long-term cognitive effects), oral Glycopyrrolate, and Iontophoresis. I wanted a solution that lasted longer than a day or a week, as I find the upkeep tiring... (thanks ADHD for making it hard to keep habits!)

I really wanted to extend the life of iontophoresis. It's already great, but it's a 2 hour ordeal. I have already tried hardening my water with baking soda, epsom salt, san pellegrino, etc but to no avail. Then, I found an article (here, scroll down) citing studies (#1) (#2) about using Glycopyrrolate in the water used for iontophoresis. These patients in the studies would see up to 30-40 days of relief after ionto w/ glyco.

These studies used glycopyrrolate solutions at concentrations of 0.01-0.1%. I use about 0.75-1L per tray for my iontophoresis, so you would need 75-100mg of glycopyrrolate to reach 0.01%. Unfortunately, I do not have hundreds of spare glyco pills (2mg each). I asked my dermatologist about prescribing high doses of glycopyrrolate for this use, citing the studies, and that they do this in the UK/Europe (I am in the US), but she denied my request, stating that only happens in case studies and such (not true lol but okay fine). Since my goals for that appointment were to 'get something new to try', she did, however, give me a prescription for Qbrexza... which is high dose topical glycopyrrolate šŸ˜. With some trial and error, and some math, I have developed a dosing guide.

To make a glycopyrrolate solution, I simply add unfolded Qbrexza wipes into my pitcher of hot water. I stir for about a minute. Next, I pour the water into my trays/cup. Then take out the wet wipes and squeeze them out over my tray, then throw them away. Then, I add the electrodes into the trays and do my iontophoresis like normal. When I'm finished, I pour the water down my bathtub drain and rinse off my trays.

Glycopyrrolate Concentration guide:

My maintenance dose:

  • 0.0264% concentration
  • I use 1 part freshly boiled water mixed with 2 parts room temp water. Warm water is supposed to enhance effectiveness. I mix the waters in a metal graduated pitcher before pouring them into my trays/cup.
  • Underarms: 0.5L water + 2 wipes for both pads, soaked in a ~8oz cup.
  • Hands and feet: 0.75L water + 3 wipes each tray. Hands first, water reused for feet. .25L hot water, .5L room temp water.

Dosing suggestions:

  1. Begin at the lowest concentration (0.0088-0.0132%) or 1 wipe per tray
  2. Wait and see how long that first dose keeps you dry for
  3. Do next treatment as soon as sweating begins again. If interval between treatments is too short, or effects not strong enough, increase the glyco concentration.
  4. Repeat until satisfied with results or side effects becone too strong to tolerate.

BEFORE TRYING - YOU SHOULD KNOW - WARNINGS:

  • Do NOT get the glycopyrrolate solution in your eyes. The same warnings about Qbrexza also apply here. It can cause pupil dilation that may last days/a week. I wear safety glasses while handling and using my solution/doing iontophoresis and don't take them off until I am completely cleaned up and everything is put away.
  • You may experience potentially strong glycopyrrolate side effects for 6-24hrs after treatment. Pay attention to your body, know your limits, and start with a low dose, increasing gradually. If glycopyrrolate gives you chest pain, dizziness, lightheadedness, fast heartbeat, etc, then I especially urge you to exercise caution when applying this technique. Do so at your own risk. While these side effects are rarer, and I do not believe they cause actual harm, it is still something to keep in mind and ideally, to be avoided. Personally, I only experience dry mouth/eyes and maybe headaches if I'm not on top of my hydration.

PROS:

  • relief for up to 4+ wks
  • save $$$ on Qbrexza (I personally only use ~8 wipes per month, instead of daily use. Qbrexza is $40/box of 30 for me so I'm saving a lot!)
  • boosted iontophoresis effectiveness

CONS:

  • iontophoresis cons (time consuming, skin irritation, machine price, etc)
  • potentially strong glycopyrrolate side effects, but only for 6-24 hrs after treatment. I get dry mouth and dry eyes, which can get very uncomfortable if I'm using more wipes, (if it's really bad I can't even eat dry foods lol) but I work around it by doing iontophoresis right before bed/later in the day. That way, I go to bed after cleaning up and wake up the next morning feeling normal.
  • YMMV... I get myself dry enough to where I only need 1mg glyco/day. My standards/threshold isn't super high. I haven't tried going without my daily glyco! And I honestly don't mind enough to try to. if your goal is COMPLETE dryness without supplemental glyco, I'm unsure how often you would need treatment... maybe every 2-4 weeks? If you wind up doing that, tell me about it! I would love to collect data and hear your experience.

FAQ:

  • Can you use crushed Glyco pills? Yes, this is what I did before my Qbrexza Rx. However, the dosing will obviously be much lower since you are limited by the dose and number of pills. I barely saw a difference between plain tap water VS water with 2-10mg glyco, hence why I switched to using Qbrexza.
  • Can you use a different antiperspirant / anticholinergic agent? Yes, in theory it should work too, but I do not have the research or experience to back it up. So, be cautious, and start low and slow. Experiment at your own risk. Feel free to tell me about your results! If you use Aluminum Chloride, do not use a metal container to hold the solution--it will rust.

Vent Update

So about 2 1/2 weeks ago, I made a post asking for advice on how to deal with this and a lot of of you all recommended that I go to the hospital, but due to not having insurance, I wanted to wait until October when I get insurance.

Well, I ended up going to the emergency room because of this constant scratchy throat, and the feeling that a small piece of food was stuck, that’s been going on for about two months.. They looked in my throat and found nothing, but as I was sitting there, I immediately started pouring sweat, and the doctorā€˜s eyebrow raised up. She asked if I had any diagnosis for the sweating and I said no it just started about three months ago. That’s when she said that she wanted to do an ultrasound on my throat.

Almost immediately the vibe in the room just went to shit. They found a solid mass on my thyroid. About 70% of it is on the right lobe and the rest is on the Isthmus. As they were doing the ultrasound, they put this really long needle in for a biopsy. She said not to worry most of the time it’s just benign, but she was gonna refer me to an endocrinologist anyways. I met with him and he confirmed the biopsy showed cancerous cells and scheduled to have some test done.

On Wednesday, I had a MRI done, blood drawn, and another biopsy done on the back of my neck, and today I met with the endocrinologist again.

The tumor is cancerous, it’s in my lymph nodes, and it spread to and found lesions on my C3 C4 and C5 vertebrae. The official diagnosis is Stage 3 follicular, thyroid cancer with confirmed metastasis

I’m gonna meet with the doctor again soon to come up with a treatment plan

I was still getting comments on my last post so I thought I would post an update if anybody was interested.

Looking for advice How do I stop sweating in public?

Hey everyone, I'm struggling with excessive sweating and it's got to a point where it's gotten out of control. I sweat so much when I'm in public, my hair starts dripping in sweat and my whole body heats up, I genuinely don't know what to do. Even when I'm just sitting down in class sweat starts pouring from my head and I get made fun of. I try wearing light clothes and use antiperspirant deodorant and nothing works. This has been going on for my whole life and it is ruining myself, it's gotten to a point where I don't even want to go out anymore because I'm just going to start sweating so much for no reason. I start school in a couple of days and I want to know how to stop sweating so much. Any tips or tricks will help me šŸ™šŸ™

Looking for advice How to deal with overheating on oral meds?

I finally found a doctor who was willing to order glyco for me (I’m from the U.K. but based in West Africa right now). I’m going to pick up the meds tomorrow but I’m nervous because I reacted very badly to oxybutynin. I had man symptoms but the one that drove me crazy was overheating during the British heatwaves

Now that I’m in a very hot and humid country, I’m stressed about feeling like an internal oven. Any recommendations on how I can minimise this? Portable fans are futile in 85% humidity

Looking for advice Palmar Hyperhidrosis

Hi, my 18 year son is really down about his sweaty hands. Dr had him try Qbrexa wipes but didn’t do much, and now put him on Sofdra 12.45%. I can’t find anything online anecdotally on if this works on hands (mainly for armpit).
If anyone has tried it and can let me know, I would really appreciate it!

Looking for advice How do you prevent sweating on the metro/bus during rush hour?

How do you deal with public transportation during rush hour?

The metro/bus can be a nightmare for me. It’s crowded, hot, there’s barely any airflow, and people are standing very close to each other. Even if I’m not doing any physical activity, I can suddenly start sweating heavily, especially on my face, hands and back.

I’m particularly looking for practical tricks that actually work in real life.

Looking for advice Just realized my resting BPM is 100

My friend had a spare smartwatch and lent it to me for a few days. Man, I’ve noticed my heart rate is around 100 BPM while sitting and goes up to 140 BPM when I’m walking. I’m thin and have been going to the gym for 10 years. Can a high heart rate like this cause full-body sweating?

Looking for advice new to drysol

i just got prescribed drysol 20%. i sweat so much under my arms and boobs. those are the only two places i really need it. some of the itching and irritation that people have mentioned with drysol is scaring me. ā€˜prescription’ strength deo a lot of times irritates my armpits even tho i dont consider myself a sensitive skinned person.

is there a spot on the body i could try it first that wouldnt be so scary to test out like underarms or under boobs?

any tips that i haven’t already read here?

i just don’t know know the level of itching i could handle??? i’m scared yall!!

Looking for advice Iontophoresis advice

I’ve been using my machine for about 4 months now. I haven’t been super consistent (taking breaks for 2 weeks or more) when I do the treatment my hands and feet are very itchy!! To the point where it’s all I think about. I try not to scratch so I don’t cut my skin, but it’s non stop. Has anyone experienced this? Do you think I need to swap my towels out?

Looking for advice Anxiety sweating and glyco

Hey guys,

I am about to try glycopyrrolate for my sweating but I want to ask about a rather specific situation to see whether glyco has helped anyone else.

Basically where my sweating gets really bad is when I find myself in anxious situations, in particular with the other people around. I tell myself I’m going to start sweating and then all of a sudden I feel like my body is burning hot from the inside out and I feel restless and obviously am dripping sweating from many places, and I know this happens all because I am anxious about the thought of sweating so I start sweating!

I really want to try glycopyrrolate for these situations but I think the anxiety from it is so bad that I genuinely cannot imagine it helping. Has anyone experienced similar situations and found glyco to help?? Also side note I also sweat a lot in hot weather or exercising too so I have more hopes about it helping for that.

Looking for advice Time for change

Hey everyone!😊 Since i was young it’s always been apparent that i sweat a lot more than the average person! I was really self conscious about it growing up, especially at school. I could be writing on a piece of paper and the ink would run because the page would end up wet through. I’d avoid touching anyone’s hand because I know they would point out how sweaty I am. I’d try and avoid doing exercise where anyone could see because I’d be so embarrassed to how much I’d be sweating when no one else had even broke a sweat! I love being social and I love being active. Now I’m 20 I really want to get to the bottom of this. I’ve been to the GP a few times and I’ve tried every roll on, spray, and cream you could think of. As you may of guessed… none work. Along with this I’d just be told ā€˜oh well you may grow out of it’. Well here I am and I’m no different. I sweat from everywhere, I don’t have a specific place but especially my face and hands I’m the most conscious about. I’ve done some research into certain medications to help. I wanted to know peoples experiences with that and any side effects that come along with taking oral medications for hyperhidrosis. I have booked a GP appointment so I can hopefully try and find a solution šŸ¤žšŸ¼it would really change my life to get this under control so I can enjoy the things I love and not have to worry all the time šŸ™ƒ

Offering advice Vitamin D2 seems to have stopped my constant palm sweating

I saw a post about how some people don't absorb D3 (The common vitamin D supplement) and I am always a bit low on vitamin D when I get bloodwork done, so I figured I would try it. I got the amazon basics D2, and after 2-3 weeks I saw a HUGE improvement. Its been around 5 months now and I only get a bit of sweating when I am in a super anxious situation, but even then it doesn't compare to before. I have been shaking hands with reckless abandon and before my hands would be soaking wet just thinking about it. I have hesitated to share the results because I am certainly not a doctor, and you should talk to a doctor before adding a new pill to your life but it certainly has helped me a ton, to the point where I wonder if this was always why I had this problem. 30+ years with the problem and I have never come across an easier solution. Seemingly no side effects, just like 95% reduction in sweat.

Looking for advice Stopping glyco and sweating like crazy

For those of you who regularly take glyco, if you stop for a few days, do you get a lot of ā€œreboundā€ sweat? I’ve been taking 2mg daily for a few years with only a few small breaks.

Today is my second day without taking it, and man it’s like someone opened up a faucet! Haven’t been this sweaty in a while.

Looking for advice Has anyone not washed off Drysol? Did it affect anything?

I recently got Drysol and I’m using it pretty much everywhere. When I sweat I sweat everywhere and it feels like I’m in a swamp I hate it. Anyways, I want to put it on my back which I’ll need someone to do it for me, but I won’t have anyone to help me wash it off in the morning. I’m usually pretty busy first thing in the morning with no time for extra stuff. Has anyone not washed off Drysol the morning after? Did it affect anything? Make you sticky, ruin your clothes, etc?

I genuinely don’t mean to sound dramatic but dealing with this problem was ruining my mental health. I was considering quitting my job because it was so embarrassing and ruining my self esteem with bad foot sweating and odor. It was causing anxiety. I felt hopeless trying all sorts of creams, powders, deodorants; even bleach for my feet and it’s only been 3 days and antihydral has my feet almost completely dry. I already feel happier- and it’s almost an odd physical feeling of having dry feet for once. Recently I damn near gave myself trench foot or athletes foot and was desperate. Thank you for those who suggested this

Treatments Please find a cure , I am devastated.

My hyperhidrosis acts like a ON/OFF switch with a continuous leak — I thought that this might help with research . I am sharing my symptoms in detail in hopes of finding better insights or potential cure. My hyperhidrosis functions almost entirely like a literal ON/OFF switch. The moment any single area starts sweating—whether it is my hands, feet, or underarms—every single one of those areas immediately starts sweating all at once. The underarms are slightly less severe than the hands and feet, but still very uncomfortable. There are times when the system is completely "OFF," meaning total dryness. However, once the sweating is triggered—whether by slight physical friction, heat, stress, or even out of nowhere with no trigger at all—it starts and simply does not stop. It feels like an open leak rather than normal sweating, staying permanently turned on until further notice. The only things that turn it off are going to sleep or extreme cold; washing my hands and feet and sitting in a freezing air-conditioned room usually resets it, though not always. Treatments like iontophoresis and Driclor make it harder for the "ON" state to trigger and reduce the continuous sweating. Winter is somewhat better, but the condition remains severe. This has been present since childhood, starting around ages 7 to 8, and it has been accompanied by excessive salivation (hypersalivation) ever since I was an infant. There is no family history of hyperhidrosis. Regarding common procedures, I will never risk ruining my body with ETS surgery. Many people seem dissatisfied with MiraDry, and Botox is temporary,extremely painful, costly, and ineffective for many, though I have not personally tried it.
Currently i use ionto its pretty good.

I shared these symptoms in detail hoping to find someone with an identical pattern, or perhaps to provide useful insight for researchers working on a definitive, side-effect-free cure. I understand that our patterns and underlying biological causes differ, and I have read into sodium channels like Nav1.7 and Nav1.8. Right now, I am at a critical, defining stage in my studies, and this condition is mentally crushing. Constantly overthinking and dreading how this will affect my future is severely taking a toll on my academic performance. I genuinely hope that ten years from now, all of this will just be a distant memory. When do you realistically expect this torment to end? While I try to stay optimistic about the rapid acceleration of medical technology, AI, and biotechnology, that optimism often fades when I remember how diabetic patients, for instance, have been told "a cure is only 5 years away" for over three decades.

Looking for advice Getting worse skin conditions?

I have always had hyperhidrosis (never even knew what my actual condition was until I discovered this community). My party trick has always been my dripping, sweaty palms.

Since about four years ago (I’m 30 now), the corners of my lips started cracking often. I initially thought it was sunburn (I used to live in a tropical country). But now I live in colder climate and my lip corners still get cracked during winter. I cannot survive without a lip balm anymore, even at home and while hydrating/drinking lots of water.

More recently, I’ve also developed dermatographism. I’ve been prescribed antihistamine medication which helped, but I don’t want to take it all the time.

I guess I’m just wondering if anyone else here experience similar symptoms? I still plan to get these checked by a doctor because I’ve always tried to treat them separately, but maybe it’s all connected?

Looking for advice About to be shaking a lot of hands soon, need advice

It happens rather randomly and I have tried the carpe stuff (didn’t work) and body powder which somewhat works but only briefly and my hands get all white. Does anyone have advice?

Looking for advice What are the best clothes to wear to the gym to avoid sweating so much?

As soon as August ended, they turned the air conditioning down at my gym, and now I sweat a lot — even to the point of having drops of sweat running down my face.

I’ve been trying breathable micro-perforated T-shirts and I’m really happy with them, although most of them are a bit ugly until I find one I actually like.

But I can’t find anything similar for shorts or trousers. What about underwear? And shoes and socks?

For now, I bring one towel just for sweat and another one to use on the machines.
