Embryo donation question

Has anyone donated embryos created with Xytex donor sperm? Xytex is requiring Form 9008.3 and charging me—the embryo donor/original sperm purchaser—a $1,195 transfer fee. If you’ve donated embryos to more than one recipient family, did Xytex charge you the fee again for each family?
