Open | Hardware Monitor showing lines

Hello, I have lines on my 27in 1440p monitor, however they dont appear on any of my other monitors.
The lines blink on and off on a 1 second cycle almost perfectly.

Monitor is set to the correct refresh rate, in both the monitor itself, and the windows settings.
GPU is up to date on drivers. latest windows updates have been done as well.
GPU is about 1 year old ish.

installed previous drivers for gpu, no change.
Issue only appears 15 seconds after logging into the computer, even when no software is on.
I unplugged and swapped my display port cabels around with my other monitors and the issue persists.
Changing the refresh rate only changes the rate at which the lines blink, lower refresh has a static lines, high refresh rate gives whats in the video.
