You're thinking of RAG (Rent A Girlfriend). The 100 Girlfriends is still horny wish fulfillment, but the main protagonist actually does work extremely hard to keep his partners happy and cared for.
I've said this every time the idea's been brought up: I hope the last girlfriend doesn't love ANY of her 99 loyal boyfriends she got through another love god's blessing, so for a final serious arc, Rentarou has to gather his girlfriend army to fight/distract them so he can help #100 fall in love with him and avoid the curse befalling her.
Also Nakamura, please more families in the main series. I love the spin-off that focuses on the girlfriends' families but I hope they appear again in the main series. An arc where Rentarou has to explain the whole "Rentarou Family" thing to the girlfriends' families will be fun.

This reminded me of a silly manga that I never picked up, but it went something like this: a girl is a really good actress (or has some multiple personality disorder?) And acts different character who are in love with her unfortunate friend, but slowly falls in love for him as a consequence of acting a character falling in love for him... really ironic.

I hope the trans girlfriend becomes real. Considering this work is already very gay, safespace included, and one of its core themes is to accept each other's quirks as something normal, it makes a lot of sense. Also might demostrate the enormous gigachad Rentaro is.

I was one of that many people that thought it's a slop with such outrageous title, but my homeboy caught up to the latest chapter and can't stop praising it so I gave the anime a 2nd chance. Best decision ever because ITS PURE PEAK I genuinely couldn't stop smiling through season 1
